Release of Patient’s Records Policy
In compliance with HIPAA regulations the Tolland Fire Department has established the following policy for the release of patient records. All requests for records must be made by mailing an original HIPAA compliant medical release that is NOTARIZED. Please state specifically what documents you require, such as Patient Care Reports, itemized bills, etc. Tolland Fire Department will only release the specific documents requested.
